The two clubs have already agreed a fee for the former Chelsea star.
Tiago joined Chelsea in 2004 for £8million from Portuguese side Benfica but in 2005 he moved to French side Lyon for £6.5million.
The 26-year-old midfielder joined Juventus for £9million last summer.
Tiago has snubbed the chance to join Spanish side Atletico Madrid.
